I hereby indicate my affirmation of Brethren in Christ U.S. theology and doctrine as set forth in the Articles of Faith and Doctrine.
My views have moved beyond those set forth in the Articles of Faith and Doctrine in the following area(s).

In applying Scriptural directives related to exemplary Godly living (See I Timothy 3:1-7; Titus 1:7-16.) the church’s statements about Christian life and practice are normative for credentialed ministers (See the Manual of Doctrine and Government). Recognizing that the Holy Spirit continually leads us forward in yielding to God, is your life and practice compatible with these norms?
(Required)
Yes, I affirm the church’s statements about Christian life and practice, and, with God’s help, have been able to exemplify these norms in my life and ministry.
No. There are aspects of my life and practice that are not compatible with these norms.
If no, please explain how you are addressing these issues.

Lifelong BIC Learning
(Required)
Lifelong BIC learning is seen as an effective way to nurture a cohesive Brethren in Christ U.S. perspective in the life of the church, and the Ministry Training offerings provide a wide range of ministry enhancing possibilities. (See listings at https://bicus.org/training/). The Leadership Council and the CMD have endorsed lifelong BIC learning for credentialed ministers and anticipate that all assigned ministers will have completed at least four offerings provided or approved by BIC U.S. during this review cycle. With this in mind, please respond to the following question. In which of the Ministry Training offerings did you participate in the past six years?

Background Check Requirement
The Brethren in Christ U.S. (BIC U.S.) requires every candidate applying for or renewing credentials with BIC U.S to submit a background check through Christian Background Checks. We require background checks of employees and licensed ministers as “due diligence” in protecting the persons and integrity of the ministry of BIC U.S. After submission of this renewal form (including the 2 acknowledgements below), you will receive an email from Christian Background Checks instructing you of your rights and seeking your authorization for a background check to be conducted. You will also receive instructions on how to pay for the background check (approximately $16.75). Do not put your maiden name in the “Alias” box, as that will double the county searches. Searches are done primarily by SSN. The Brethren in Christ U.S. will follow the requirements of the Fair Credit Reporting Act in conducting background checks and may use the information obtained from the resulting report for decisions related to the applicant’s credentialing. BIC U.S. will not discriminate against the applicant or misuse the information as provided by any applicable federal or state equal opportunity laws or regulations, and in accordance with the Brethren in Christ Manual of Doctrine and Government.
Acknowledgement
(Required)
I acknowledge receiving and reading the above notification.
Acknowledgement
(Required)
I understand that in accordance with the federal Fair Credit Reporting Act, if any adverse action is to be taken based on the background report, a copy of the report will be provided to me. A summary of the candidate’s rights will be provided by Christian Background Checks at the time of authorizing the background check to be conducted.
Feedback Opportunity
The CMD would welcome your suggestions or input with regard to this periodic review process and ways to maintain ministerial accountability among us.
